UNBELIEVABLE how these containers BREAK OFF and FALL into the ship hold. Sleepy crane operator forgot to activate twin mode and MAKES THIS MESS...!? Stupid Stupid Stupid!!

Crane operator did not activate twin-mode (2 container mode)
Hatchman did not warn crane operator
TTDS sensor failed: this laser can detect 2 20" containers, spreader will not lock unless crane operator push a button!
